Controversies in Mediastinal Staging of Peripheral Lung Nodules: Should We Perform EBUS at All?

Description
The question of when to pursue invasive mediastinal staging with EBUS in the setting of a negative nodal evaluation by PET has been a frequent topic of debate. There is no clear consensus between ACCP, ERS, and NCCN guidelines on this topic. ACCP guidelines, suggests that peripherally located radiographic stage 1A disease (ie, less than 3 cm) should not require EBUS staging. A recent publication in 2017 suggests that nodal metastatic disease for peripheral T1aN0 NSCLCa is not negligible. In this Pro-Con debate, we present two well-known experts debating the utility of EBUS in all these scenarios.
